District Band Audition Registration Form


Deliver all materials to:

Audition fee is $3.00 per entry.  A student may audition on one instrument only, with the exception of
percussionists who may audition on one, two or three instruments, in which case they must pay the $3.00 for each
audition and be counted for each audition in the totals below.

YOU MUST TYPE A LIST OF STUDENT NAMES AND CORRESPONDING INSTRUMENT ON THE ENCLOSED FORM AND SUBMIT WITH YOUR REGISTRATION.

We certify that all students represented on this form are regularly enrolled in our school’s band program in the
grade level required for eligibility.  We agree that this form, Pre-registration Form, all fees and photo copy of the
director’s MENC card will be in the hands of the Audition Chairman on or before the deadline date;  we
understand that these students will be ineligible to audition if our entry arrives after the deadline or if the director
fails to fulfill his duties on the Audition date.  PRINCIPAL’S AND DIRECTOR’S SIGNATURE ARE
REQUIRED.
